ACL Reconstruction Surgeon in Mira Road, Mumbai
An ACL tear that is missed does more damage than the original injury. This page covers how to recognise one, why an X-ray cannot rule it out, whether every tear needs surgery, how graft choice is made, and an honest return-to-sport timeline — from a surgeon fellowship-trained in arthroscopy and sports surgery.
The Injury
How an ACL Tear Happens — and How It Is Missed
The anterior cruciate ligament sits inside the knee and stops the shin bone sliding forward and rotating on the thigh bone. Most tears are non-contact: a sudden pivot, a landing, a change of direction with the foot planted. Cricket, football, kabaddi, badminton and gym training account for most of the cases seen locally, and a significant number come from ordinary slips and two-wheeler accidents rather than sport at all.
The classic account is a pop or snap at the moment of injury, immediate difficulty continuing, and a knee that swells substantially within a few hours. That rapid swelling matters diagnostically: it usually means bleeding inside the joint, and bleeding means a significant structural injury rather than a sprain.
Why ACL tears get missed for months
This is the part patients are rarely told. After a few weeks the swelling settles, the pain largely goes, and the knee feels serviceable for walking in a straight line. Many people are reassured at this stage and told it was a sprain.
But the ACL does not heal by itself, and the knee is only stable while it stays on predictable ground. The tell-tale symptom is giving way — the knee buckling when turning, stepping off a kerb, or moving on uneven surfaces. Each of those episodes is the shin shifting on the thigh, and each one can tear meniscus and damage the joint surface. A patient who lives with an unstable knee for two years often arrives with a meniscal tear and cartilage damage that were not there at the time of the original injury — and those are what cause arthritis later, not the ligament tear itself.
The Decision
Does Every ACL Tear Need Surgery?
No — and this is a genuine decision, not a formality. The right answer depends less on the MRI than on what you need the knee to do.
Surgery is usually the right call
- You play, or want to return to, any pivoting sport — football, cricket, kabaddi, badminton, basketball
- The knee gives way in daily life, not only in sport
- Your work demands stability — uniformed services, physical trades, long hours on uneven ground
- There is an associated meniscal tear worth repairing, since repair works best alongside a stable knee
- You are young, with decades of loading ahead of the joint
Rehabilitation first is defensible
- Older, lower-demand patients whose activity is mostly straight-line
- A knee that has never given way after a properly supervised rehabilitation programme
- Partial tears with good stability on examination
- Where a structured strengthening programme has genuinely been completed — not merely advised
Where surgery is not chosen, the commitment to quadriceps and hamstring strengthening has to be real, and the knee needs monitoring. An unstable knee that keeps giving way is not a stable compromise — it is ongoing damage.
The Surgery
ACL Reconstruction, Graft Choice and Getting Back
A torn ACL is not stitched back together — it is reconstructed. A graft is used to build a new ligament, passed through tunnels drilled in the thigh and shin bones and fixed in place. The procedure is arthroscopic, performed through small incisions with a camera, usually as a short admission.
Graft choice
Most commonly the graft is taken from your own body. Hamstring tendon grafts are widely used, leave less trouble at the front of the knee, and suit patients who kneel frequently. Bone–patellar tendon–bone grafts offer very secure bone-to-bone healing and are often preferred in high-demand pivoting athletes, at the cost of more frequent kneeling discomfort. Quadriceps tendon grafts have become increasingly popular as a middle path. Each is a reasonable choice; the selection should follow your sport, your work, your kneeling requirements and your anatomy — and your surgeon should be able to explain why yours was chosen.
Timing
Operating on a stiff, swollen knee produces worse results. It is usually better to spend two to four weeks first restoring full straightening and settling the swelling — called pre-habilitation — and then reconstruct. The exception is a locked knee from a displaced bucket-handle meniscal tear, which needs earlier attention.
Return to sport is measured in months, not weeks
This is where expectations most often go wrong. The graft is at its mechanically weakest around six to twelve weeks after surgery, precisely when the knee starts to feel good — which is why the most common cause of a failed reconstruction is returning to sport too early. A realistic pathway: full straightening and walking without aids in the first few weeks; strength and control work through months two to five; running from around three months; sport-specific and cutting drills from four to six months; and return to competitive pivoting sport usually at nine to twelve months, gated on strength and hop testing rather than the calendar alone.

Locally
Arthroscopy and Sports Surgery in Mira Road
Dr. Avinash A. Kachare holds a dedicated fellowship in arthroscopy and sports surgery in Hungary, alongside an MS in Orthopedics from LTMMC & GH (Sion Hospital) and fellowships in robotic joint replacement and 3D printing at Queen Mary Hospital, University of Hong Kong. Arthroscopic work at the Mira Road practice covers ACL and PCL reconstruction, meniscal repair and partial meniscectomy, cartilage procedures, and shoulder arthroscopy for rotator cuff tears and recurrent instability.
Consultation and follow-up are at Surgicare Plus, Poonam Cluster 2, Mira Road East, with surgery at associated hospitals in the area. For younger patients in particular, having reconstruction and — more importantly — the year of follow-up and rehabilitation available locally rather than in town makes the difference between a programme that is completed and one that is abandoned halfway.
